The Kentucky HIV/AIDS Care Coordinator Program (KHCCP) is dedicated to ensuring individuals and families affected by HIV receive timely, consistent, and high-quality care and support services. Operating through seven regional sites, the program offers needs assessments and service coordination to guarantee statewide access to essential local resources.
Purpose of the Kentucky Care Coordination Program
The core purpose of the Kentucky HIV/AIDS Care Coordinator Program (KHCCP) is to facilitate prompt, consistent, and continuous quality care and services for individuals in Kentucky living with HIV and their families. Care coordinators, situated across seven strategic regional locations, are responsible for evaluating individual needs and connecting clients with necessary services. This regional approach ensures that people throughout Kentucky can access support within their local communities.
KHCCP serves as a central program, encompassing client assistance initiatives such as the Kentucky Health Insurance Continuation Program and Outpatient Health Care and Support Services. Funded primarily through federal sources, KHCCP acts as a payer of last resort, emphasizing that direct financial aid is not guaranteed and is subject to funding availability. It is important to note that due to funding limitations, service availability can vary by region and may be adjusted or terminated as funding levels fluctuate at both the state and federal levels.
Goals of the KHCCP
The Kentucky Care Coordination Program is structured around several key goals aimed at empowering clients and streamlining service delivery:
- Enhancing Self-Sufficiency: To maximize clients’ abilities to manage their own care by fostering independence and self-reliance.
- Facilitating Support Systems: To establish effective coordination between clients’ personal support networks (including family, friends, caregivers, and counselors) and the formal support systems available through healthcare and social services.
- Improving Access to Services: To guide clients in identifying and accessing a comprehensive network of referrals for critical services such as medical care, housing assistance, language services, Medicaid and Social Security Disability benefits, transportation solutions, and legal aid.
- Preventing Service Duplication: To minimize or eliminate any overlap in services provided by both formal agencies and informal support systems, ensuring efficient resource allocation.
- Promoting Health Education and Prevention: To equip clients with vital information regarding HIV transmission and health maintenance, promote healthy lifestyle choices, and offer ongoing case management for secondary prevention efforts.
- Identifying Needs and Advocating for Resources: To recognize and document trends in service needs, and to advocate for the development of effective policies and resources that address these identified needs.
- Ensuring Efficient Fund Utilization: To guarantee that program funds are utilized efficiently and effectively to address the needs of individuals living with HIV, through the strategic coordination of funding streams and the optimal use of existing service infrastructure.

Kentucky Health Insurance Continuation Program (KHICP)
The Kentucky Health Insurance Continuation Program (KHICP) is designed to provide financial assistance to eligible individuals at risk of losing their health insurance coverage due to HIV-related conditions. This program helps maintain continuous coverage for those who might lose employer-sponsored benefits or private health insurance.
Eligibility requirements for the Health Insurance Program include:
- Possession of a health insurance policy that includes Medicare Part D or a prescription rider. Individual policies must explicitly cover HIV-positive status.
- Family policies must extend coverage to at least two HIV-positive family members.
- In certain cases, Part B or Ending the HIV Epidemic funds may be used to cover premium costs if deemed cost-effective and if the participant meets specific state income eligibility criteria and cannot afford premiums independently.
Kentucky Outpatient Health Care and Support Services Program
These programs deliver a broad spectrum of community-based support services, encompassing both medical and non-medical aid. This includes access to physical and mental health care, housing solutions, nutritional support, and transportation assistance. Priority services are determined periodically based on input from clients and care coordinators, needs assessments, resource availability, client satisfaction feedback, funding constraints, and other relevant factors.
